CHICAGO – The Illinois Department of Public Health (IDPH) announced that it has posted the 2024 Third Quarter Report of Nursing Home Violations. Details on the 312 violation reports completed in the third quarter of 2024 may be found HERE on the IDPH website. One of those fined was St. Paul’s Senior Community in Belleville. The four facilities listed below were cited with type “AA” violations of the Nursing Home Care Act during the third quarter of 2024, the most severe type of violation. An “AA” violation is cited when there is a condition or occurrence at the facility that proximately caused a resident’s death. Click on the links below to view the completed “AA” violation reports.
• Alden Lakeland Rehabilitation & Health Care Center, a nursing home in Chicago, was fined $50,000.
•ALIYA on 87th, a nursing home in Chicago, was $50,000 for an “AA” violation and $25,000 for an “A” violation with a total fine of $75,000.
• Allure of Pinecrest, a nursing home in Mount Morris, was fined $50,000 for an “AA” violation and $2,200 for a B violation with a total fine of $52,200.
•St. Paul’s Senior Community, a nursing home in Belleville, was fined $50,000.
